Dear Eastern Connecticut Ear Nose & Throat and the Center for Facial Cosmetics Patients,

We would like to begin by thanking you for the trust you have given us over the years as your ENT physicians. Taking care of you has been an honor for our staff and all the physicians. Your continued wellness and health are priorities for us.

We are writing to inform you that, due to retirement, all locations will be closed effective May 30, 2023. Dr. Culviner will be relocating to another ENT group in Southeastern CT on May 19 th . We will be available until May 30,2023 for your urgent care.

After May 30th you can reach our office at 860-456-0287 with any billing questions or to arrange pick-up of medical records by July 31, 2023. We will not be available for medical questions.

It is important that you continue with appropriate medical care; therefore, you should establish contact with another physician at your convenience.

Medical record authorization forms will be available upon your request. A copy of your medical records can be forwarded to you or the physician you designate. If you currently use MyChart you can access your medical records, there and print when needed. If you do not use Mychart, you can sign up for it to obtain easy access to your records. We can send you a MyChart invitation. Please rest assured that our office will remain available to promote a smooth
transition and continuity of care.
